Hello,

I created a satisfaction survey form (5 questions) and want to send to people outside my organization.  Under who can fill out this form I want people outside of my organization, record name and only one response per person. Any help would be greatly appreciated.

@grosen2021 as the resp[onders are outside your organisation the only way you can get their names and email address is to ask questions for these. It is not possible to limit these responses to only 1 per person as Forms has no way of knowing who the responders are.
