Is it possible to send MS forms over outlook signature

We had a requirement for creating a Customer Satisfaction Survey in form of a emoticon and share it over outlook signature.

Is this something possible using MS forms or MS Forms PRO?

@Sakti_Mishra Forms Pro has been called Customer Voice for the last 4 years and requires Microsoft Dynamics. With Forms I guess you could add a single emoticon to an email and add the link to complete the form.
