Help with word doc

Hello, 

I had a word doc saved. I have been using the same doc for a number of different documents. It was saved with date and had boxes on it. Today when I tried to open it, all the data and boxes are gone for every doc I saved. Any help would be appreciated.

@jrbmorr Can you upload a copy of the document so that we can investigate the problem.
