Why can’t my guest in teams not access Microsoft Forms on their phone that I add to the team?

@Cassie_Yeager have you restricted your Form to "Only people in my organisation can respond"? If so then the fact that they are a member of your Team will not allow them to access the form if they are outside your organisation.
