Offset and match function

I use the following to return a name from column BF5. This is from a match (1) in column BQ6:BQ17. The result is returning 0.

The function is exactly the same as another spreadsheet and this functions OK.

I am at a loss as to why it is not functioning in the present spreadsheet.

Could you attach a small sample workbook demonstrating the problem (without sensitive data), or if that is not possible, make it available through OneDrive, Google Drive, Dropbox or similar?
