Hi facing this Issue since early this Morning. Please look into the error

Apologize for the service reliability issue . Microsoft Forms team is actively working on investigating this and in the process to identify a proper mitigation plan. We have created a discussion thread (Microsoft Forms - "Something went wrong" Error) to keep you updated. Thank you for your patience as we work to restore the Microsoft Forms service. It's our top priority to get this issue addressed as quickly as possible.

This issue should have already been mitigated; you should be able to access the form.

I to am getting this error.

Mbro_College_admin if you are the tenant admin of the form owner, please open a ticket with support team to see if we can recover the form. Please provide FormID, userID and tenantID with the ticket.

Yao Ke Hi, I am also having this same issue - I accessed the editing link and the completion link three days ago but am now receiving this error message.
These are the technical details:
Session id: d0b5424a-e793-4c55-9281-bbdd9f17ea59, Correlation id: 0d84a16c-c6e7-4897-9000-b8197b1cadaf
The userID is danielle10.smith, tenant ID is northumbria. I'm not sure what the formID is.

This issue seems to be present site-wide today, and appears across different browsers (I have tested IE, Edge, Firefox, and Chrome on Windows 10).
Any assistance would be appreciated.

Hi JACQUELINE,
Could you check with your tenant admin if they enabled the Microsoft Forms for you? It seems you don't have the license.

Seems I have a similar issue as well. I can access Forms sometimes, but the majority attempting to access Forms results in "Sorry, something went wrong".

This is not working for me. No error code on web page just generic 'Something went wrong'.
From trace data I see:

Hi Brian,
From the log, it seems a dependent service (CollabDBService) was disabled in your tenant, so Forms cannot be used. To resolve this, please ask your Admin to follow following steps:
- Login to azure portal (https://portal.azure.com).
- Click "Azure active directory".
- Click "Enterprise Applications", input "CollabDBService"
- Click “CollabDBService” that is displayed in the result list.
- Click "Properties" under “MANAGE”.
- Set the option Enabled for users to sign-in to Yes. Then Save.
After that, you should be able to use Forms.

Today I started recieving an "Something went wrong" error when visiting my forms. I don't get an 631 error, but error code 1003. Is there something I can do on my end to fix this?

Hi Simon, is this issue still there? It seems a transient issue from our log, if this issue still exists on your side, can you help to provide the formid so I can help to identify the root cause. Thanks

This is no longer an issue. It was resolved yesterday around noon (GMT+1). Everything seems to work fine now.

I spent a lot of time developing questionnaires AFTER I read that Microsoft had resolved the problem. Each time, it has failed and it has been professionally embarrassing - as my clients simply give up.
This should be better for this purpose than any Adobe Product...but only if it works.

We've fully restored the service for all Forms users and will close this discussion. Summary of impact: Between 00:50 GMT and 11:15 GMT, a significant number of users encountered "Something went wrong" errors. Root cause: Architectural change to switch Forms' connection to SQL Azure. Mitigation: Engineers checked in a fix to revert the code change and flushed impacted caches. Next steps: Engineers will continue to investigate and prevent future occurrences. We have the same problem now. Our users can't acces the form.
Forms are Completely Down.

The Forms Designer has just gone down again, for us, within the last 30 minutes. Is this likely to be an on-going issue or do we have a time-scale for resolution?

Hi Justin, is the issue still there? Can you share us the correlation information (When you see Something Went wrong, click details to get the correlation information) so we can do more investigation, thanks.

I am still facing this issue, not all users are able to open the link to the form , could you please provide a work around or share a solution?
