Apologize for the service reliability issue . Microsoft Forms team is actively working on investigating this and in the process to identify a proper mitigation plan. We have the same problem now. Our users can't acces the form.
Forms are Completely Down.

Just want to check, did you try the sign-out and sign-in? will that help on this issue? We are working on diagnose the root cause, before that, please help to try if this will help to unblock you to use Forms.
