Hi, I use Microsoft 365 and use FORMS on the web regularly to create results FORMS for friends to fill in the form to show their 5k run time for the week. Over the weeks I'd created about 11 of these and they had always worked fine. However yesterday all the FORMS I had created started to disappear, to the point where they have ALL now disappeared! Frustrating as there was data in there I needed and the lasted one was in the process of being used to collect times. Has Microsoft updated something or is there a wider problem? Where have my FORMS gone? I have checked the recycle bin and they are not there! I did also notice that if I tried to fill in the FORM myself, I got the unusual message of "This form might not be safe - We recommend you don't provide your personal or sensitive information. The form owner may potentially be trying to collect it." You could then click on "Close This Page" or "Continue anyway (not recommended)" - see attached screen dump. Thanks in advance for any help. I have Form that triggers a flow when submitted. That flow puts the Form data into a SharePoint list. There's a couple questions on the form that consistently get brackets ["answer"] around the answer and sometimes the same question will have the brackets and a \n at the end like ["answer\n"]. There's another question that inconsistently gets a <br> put at the end of the answer answer<br>. I've tried deleting the questions with the problem(s) and re-creating them, but the issue persists. The problem seems to lie within Forms and not Flow or SharePoint because when I check the response details within Forms I see answers getting the extra stuff it shouldn't get. I recently printed some leaflets with the QR code and URL to my forms and had 40+ responses. I had made some minor changes to the form and the outcome of the blue when you scan the form you get an error message that read "the form does now exist".... The thing is ,it does exist. It's not in the deleted bin it's active, it's just upon further exploration I can see the QR code and URL has changed. I have been using the tick box that says make URL shorter...could it be that? Hi All, We are using MS Forms and have embedded the QR Codes into PDF documents for Customers to scan as they enter the premises. This was working well until yesterday but, now, when Customers scan the PDF it says the Form does not exist. I verified the Form does exist and checked the QR Code generated under 'Share'. The QR Code is different to what it was before. Any ideas why this would happen? One of the Forms was updated last Wednesday but was working fine up until yesterday. Anytime someone external submits a MS Form, it shows as anonymous in the email notification. There needs to be an option to show the name or first name, last name field on the form or have the option to send a notification with a summary of the individual responses. This is helpful for when I'm collecting responses for job application candidates across multiple different roles/forms within a company.
