Have you tried to combine the permission "Add items" first with "Edit Items" and then with "Manage lists" and see what happens?

Yes, tried to create Permission Level with "Add Items", "Edit Items" and "Manage Lists".However, when attempting to create a new item, after clicking the Save button on the custom form, an error appears.

Could you please select below permissions as well.
- View Application Pages - View forms, views, and application pages. Enumerate lists.
- View Pages - View pages in a Web site.
- Open - Allows users to open a Web site, list, or folder in order to access items inside that container.
