Error 633 'Sorry, something went wrong'

Hello, a form that was previously working is no longer functioning. I am unable to view the form on the back-end, and no one seems to be able to use the form on the front-end. This is an issue solely...

    Our third party tech support went through Azure and fixed something in the database. Hopefully that leads you in the right direction!
