LookUp from another list dependent on values on another cell

Good afternoon - 

I have created multiple lists - I have a flow created to generate a new item on my 'Site List' when an item is created in my survey list (This works!!!) -- however, on my Site List -- I need to look up the 'Title' and 'Region ' based on the 'Site State/Province' value. Is this possible?

with SharePoint OOTB cascading lookups are not possible. But you can user Power Apps to build cascading lookups, here you can find a great documentation:
