The telemetry indicates that the user does not have any valid Forms licenses. If the user has recently been assigned new licenses, please ask them to access the forms through https://forms.office.com/Pages/DesignPageV2.aspx.

    From the telemetry, it appears that the license status is not valid. For example, the license assigned to that user may have been disabled. You may want to contact your IT admin to ensure this.
