Microsoft Forms - Add-in Error

On a specific domain/computers we are receiving the following error when going through a powerpoint doc that has a MS Form attached to it on one slide (OWA):

Works fine on Computers that are not on this specific domain, come up with 'Trust this add-in' and proceeds to open:
