Failed to upload a file (ios ipad issue)

Hi, wondering if anyone can help with this.  We are using MS Forms internally and we have an issue with our ipad users not being able to upload images to the forms.  It works flawlessly on desktop but for ipad users we have an error saying file can not be uploaded.  Here's what I have tried so far:

1) Set the max file upload limit to 10 in MS Forms

2) Set file size limit to 100 MB in MS Forms

3) Ticked all file type options in MS Forms

4) Checked camera settings on ipad and changed file type to most compatible from HEIC for images.

Is there anything I am missing?  Anyone any thoughts as to what the problem may be?

If you are able to get the error session ID or correlation ID, please share it with me. Otherwise, if possible, you can send me a private message with your form ID and the timestamp of when you experienced this issue. I can help to check if we have any clues from the backend telemetry.
