Broken icons in Document Library
We're having an issue where the icons that appear in the Document Library show up as a '?' sign rather than the actual icon. This issue started some time last year.
Has anyone else had this happen? Is this a known issue, or is there anything on our end we can do to fix?

Do you use a column or view formatting?

Thanks for responding.
It's been happening on a handful of websites. While we don't have any formatting, I did realize that we use a custom plugin on those sites to add functionality. I'm thinking that may be causing an issue somehow.
