Thanks for responding.
It's been happening on a handful of websites. While we don't have any formatting, I did realize that we use a custom plugin on those sites to add functionality. I'm thinking that may be causing an issue somehow.

Just wanted to confirm that it was, in fact, an issue with the plugin. We use a custom plugin for some visual changes, which was interfering with the CSS. After making some changes, the icons are once again displaying properly.
