"Record Name" Option not Working Properly

I set form to acquire some information from students with enabling both options "record name" and "Only people in my organization can respond" . But form capture names of some respondents and while showing emails of majority respondents in the column of Name.  Can anyone tell me what was wrong? and How to fix it?
