I have been getting this for All the forms I create. What causes this?
This form can't be distributed as it is asking for personal or sensitive information. Contact your admin for assistance.
kavixx that's usually caused if your text anywhere in the form includes the word password or if you ask for a phone number. It seems a bit random though. If you can post what your questions are that might more of a clue.

- Thanks. Any use of Username and Password gives this error. Was trying to create an assessment on Network Security!
