Form now wont open showing Sorry, Something went wrong
Working on a new form and now the form won't open and displays this.

I'm from Microsoft Forms team. Your form has been repaired. You can verify now.
We have found the root cause of this issue. We recently added a new question type Likert. If an user adds it, then deletes it quickly (like in 3 seconds), there is a small chance that the form would run into an issue.

The form is very important and it cannot be opened all of a sudden since yesterday. It says 'sorry something went wrong, this form doesn't exist' please help me with this as this is urgent.

I have issues with a microsoft form which I'm using at my work to collect responses from my colleagues across the company I work in.

I created a form recently and it wont open now. When I open it with my account there is an error showing up as seen in screenshot below.
It is still possible for me to open and look at the answers on Analysis page, since I have copied its link to be on the safe side, but I can only see some of the results on it ( Only in graphs on some of the questions)
Could you please help recover the page?

hi there,
a few days ago i created a questionnaire using forms for my team. my team can still access and answer the questions but in my account i cannot see their responses. when i click on the form it says 'something went wrong'
can you please advise as its really urgent and i have to finalise inteviews on the basis of that questionnaire.

MS Team fixed mine. They said that my https://lists.office.com was disabled on my apps.
From the log, it seems a dependent service (CollabDBService) was disabled in your tenant, so Forms cannot be used. To resolve this, please ask your Admin to follow following steps:
- Login to azure portal (https://portal.azure.com/).
- Click "Azure active directory".
- Click "Enterprise Applications", input "CollabDBService"
- Click “CollabDBService” that is displayed in the result list.
- Click "Properties" under “MANAGE”.
- Set the option Enabled for users to sign-in to Yes. Then Save.

I am having the same issue:
I have to forms in the same Teams that is giving this issue to all 4 members in the group. I can get to the data by going to Teams and then look at it in sharepoint but the forms aren't working.

Kathy Liu I'm getting the same problem with the "Something went wrong message". I created the Form in Forms Pro, but anyone trying to open it in Forms gets the error. We also have the problem that the form shows as having 2 responses, but when opening it (in Forms Pro) it says there have been no responses. Please could the Forms people investigate.
