Approve/Reject missing in SharePoint Online document libraries

This is a bug, which I among others have reported to Microsoft. It is being fixed as I write this :-)

But do you mean Approval for a regular document library or for the site pages doc. library? I think the Approve/Reject/Publish options are only avaiable in the Item menu, but not in the doc. library actions bar
This is a regular, modern, Document Library with Approvals enabled under Versioning Settings
