Approval Process - Open Tickets

I don't really know where to start but I want to create an approval process for member submissions. 


I member started to fill out a form and based on the form input that will decide what new fields populate. 


As an example, a member enters 'red' and certain fields populate, but 'blue' on a predetermined selection will populate other fields. 


The second part of this, ever member enters their office ID and based on that office ID they are able to see their open/closed tickets. Is this possible?

You can do this using Microsoft PowerApps where you have an input form and then save it to SharePoint list
