I have created 13 separate forms. I have tested the forms and now trying to collate response data. The issue is that the name on the output excel document is wrong in many of them? And also includes some fields that aren't in that form.
Any ideas?? I did duplicate some of the forms and amend them, would this have impacted them?

@MichelleLussick yes it is most likely that duplicating the form has caused this. Do more manual adjustments and check the spreadsheet behind each form.

Thanks for your response. After a lot more playing around I did a "sync all responses to a new workbook" which has resolved the issue.
