Multiple MS Forms created - output data names incorrect

MichelleLussick yes it is most likely that duplicating the form has caused this. Do more manual adjustments and check the spreadsheet behind each form.

Thanks for your response. After a lot more playing around I did a "sync all responses to a new workbook" which has resolved the issue.
