ITEM REQUEST TRACKER WITH SUMMARY

I would like to know if anyone has a template or link of a form where i can input details of a request (table) and on the second sheet is the summary of those requests which I can sort by its column title. Thank you so. much in advance for you assistance!!!

Column titles are:

Date

Time

Name

Room

Requested item

Assigned department

Assigned runner

Status
