Version and UIVersionString Label are not always the same

I recently setup the manual UIVersionString Label so that the version of documents can be shown inside a document file from a SharePoint library.

However I have found despite doing this, the label doesn't always show the correct version and is sometimes a couple of versions behind. What can cause this?
