Chris Cundy I tested something out with an excel table and a microsoft form that with the help of a power automate would fill out the required fields but this didn't work so we are now looking at some more "out of the box" systems.

meganpat149 you could build a flow in Power Automate to get the form response and save the details into a SharePoint list. But to have a matrix front end you really are looking at building this in Power Apps.
