Update on this, the following steps were taken to fix the issue:
Reindex site
Reindex affected library
Check site and site collection result sources via settings, make sure default is set to Local SharePoint Results
In this case, somehow the site-level result source default was set to Documents, which explains why results across different site collections were showing. Still plenty of other odd behavior that wasn't quite explained (like News showing from other sites... it was always the same News items over the course of a week), but following these steps got things working as expected.
