SharePoint Document Library Request Sign-Off comments not displaying

I am attempting to use the native SharePoint feature in a document library... Select the file, and then select Automate and select Request Sign-Off.

The process works fine accept for the following.

When the user receives the message, they select the accept option and fills in the comment box.

The document is posted to SP as approved but the comments are not displayed.

When selecting the document and then looking at the information section on the right side of the screen, under "Activity", SharePoint displays "this item has no comments".

How do I view the comments provided by the user who approved the document?
