    I have worked though this problem. It is not 100% without problems however it is getting the job done for now. First:

    I go the list and copy the new form URL.

    Then I go to the page I would like to embed it.

    Then I paste the URL but I need to adjust it. I remove the source information as when I click submit the list shows in replace of the form until refreshed. I paste the new form URL in the source area as well. This seems to walk.

per use license and our IT department says once the form is created there is a charge for each person who uses the form each time, they use it. ganeshsanap
