'Name' Field Missing in Default Input Form: How to Fix It?
When adding a new item to the list, the 'Name' field does not appear in the input form. The 'Name' field is not hidden, and the input form is set to the default form.

i think name it's not a default column, but title. However, you can edit the columns, they will show up in the list form. Add a new item > Edit columsn (in the top right corner) and select the columns.
