Hi all, can anyone suggest me how to create this type of sharepoint form view in sp list?

right now i am facing NAN error & different dates in dates design view. Because we are using DD-MM-YYYY format.

i attached a document to it....in that code the output will be in the form of MM-DD-YYYY.........where i have to change
