Files moving unexpectedly
We have a document library that contains many folder, each folder contains Document Sets which contain document files.
Recently we started seeing files show up in the top level of the library. They appear to have moved (on their own) out of the document sets and folder in which they were located. We are speculating that this might be related to the new sync client, but don't have any evidence to support this idea.
Has anyone else seen anything like this or have any ideas what could cause this?

I've seen this behavior both in SP2010/2013 and written a possible workaround in this blog post:

What specific offline sync client are you using?

Thanks for the tip and post, I have shared that with the site owner and we are trying to track down all of the site members to investigate their sync clients
Were you able to diagnose and fix this issue? I have a client with the same thing occuring.
- I have not seen this...is it happening on modern UI or in the classic one?
- We are on First Release, but that library is still in Classic mode because of some issues with the Content Types
