Difficulty with calculated column

Hello,

I am having issues with a calculated column where I am trying to adjust a date based on a defined frequency.  I have a column set as a numerical value for number of months that I would like to add to a date but when I put the formula in my output is not what I would expect.  Below is an image of the columns I am using as well as my formula.  Thank you for any support.
