Is there a LOP List/Question list App in App store?

Hi All

I was wondering, wether there is any App for tracking Request for information - ROI / LOPs / question lists?

Basically my intention is to have a common platform where either my client or I can submit questions, they can answer it, maybe attach something, in the most easy way. So far we just used an excel sheet where the questions were listes below each other and right next to them we had the answers.

Would be nice to have such thing as a tab on my channels, which is not just a simple excel sheet anymore. (not looking for tickets, because I think there would be too much questions)
