Thanks for your reply. I did try googling before posting here.
Unfortunately after ~1h or searching and studying Polly or Forms, I couldn't see how I could achieve my goals with their functionality.
So, I decided to ask here.
Note that I am looking for the listed 5 features for polls which are not some default features.
