Continuous feedback in Teams without it getting lost in chat?

Our company is trying to move from annual reviews to continuous feedback. The idea is that managers and peers give quick feedback throughout the year and it all gets captured somewhere. Right now people just send feedback in Teams chat or email and it vanishes. By the time reviews come around nobody remembers what was said 6 months ago. Is there a structured way to give and receive feedback inside Teams that actually saves it and makes it retrievable later?

    This is definitely the most common way to approach feedback in remote organizations. Another way I've seen people handle it is through creating specific feedback templates in word or excel and just having a folder in sharepoint. These are are all pretty messy and they don't give the feedback process the respect it deserves. There are some amazing third-party feedback software in the Teams app store that you can try. Some of them fit Teams so well, you might even forget you're using an integration. I keep hearing great things about Teamflect when it comes to feedback, goals, and other HR adjacent processes. You can definitely give them a look.
