We've looked through plenty of different ways to do it. For the longest time, we would keep word documents in sharepoint with everyones reviews. A partner recommended Teamflect and it fits the whole reviews in Teams thing really well. Its been smooth for us. I'm sure there are other great apps in the store too. Id recommend looking for third party options. Manually it becomes very disjointed very quick.

Typically you send the employee a document which asks questions such as accomplishments, what have they learned during year, etc. Manager fills out same or very similar. The 2 meet in Teams video 1 on 1 and discuss what each wrote. Once come to an agreement both parties sign off on the document, gets routed to HR and then the employee receives their pay raise and bonuses.
