This form can't be distributed as it is asking for personal or sensitive information
Ive had this appear out of the blue and the Form has been live for the last 2 months?
There is no information to show what information I have created that is personal or sensitive.
This is for a registration form I have created for my old Collegian.
Can someone help here? I cant access any data.
- I think Microsoft broke something. They need to fix it asap.

the form can be unlocked by a global admin, by logging into Admin>Health>Message Center
You will need to read the message to find the link Forms admin review URL - VikasTrainer1Copper ContributorI work with a university.
We are not able to retrieve the responses to an assessment that we conducted for students. The error displayed is: "This form can't be distributed as it has been tagged for phishing."
Please help us resolve this, as we need the scores of the students urgently.
I have tried editing text fields, adding text fields etc. There were some math questions based on interest calculations which had the words 'bank' and 'account'. I have removed those words too. But the form is still locked.- Viviana_Mascardi_GenoaCopper Contributor
Hi all, I'm Viviana Mascardi from the University of Genova and I run in the same issue as other university professors: I used Microsoft forms from my professional account to collect proposals for a project, the form included the word "code", since it's a computer science project, and required the name and email to be inserted. I tried to remove "code", should this word suggest that I'm asking for some personal code, and I removed the text requiring name and email. There are no asterisks, there are no mentions to "bank", "account", etc. Still, the form is blocked and I cannot download the students' proposals, that I must assess in the weekend.
Can anyone in Microsoft fix the issue and explain what might have triggered the wrong tag of phishing?
Thank you, Viviana

- jean-paullizotteCopper ContributorHoping this will help, it did for me:
Edit the form: make sure any of the questions don't have the word "password" in it (if so, change it for something else). Make sure none of the questions contain consecutive asterisks, if so change them to another character.- VikasTrainer1Copper Contributor
Thank you Jean.
I checked for the word password earlier and once again now. I also checked for ** in the form. None of these are there in the form. I have also tried removing any other terms such as currency symbols; words like 'bank', 'account', 'code' etc. which were a part of Math and Logic questions in the assessment.
However, the form is still locked. Thanks a lot for responding.
- jean-paullizotteCopper ContributorFWIW if you have a word or a phrase surrounded by asterisks in your questions, it will also trigger a false positive.

Team, Can any one help here? even am also facing the same issue
- WhatsHerFaceCopper Contributor
bharath1555 look for the word "password" in your form - that's what I had to remove from mine in order for it to work
- Matlot139Copper ContributorThis is ridiculous. It's still happening to forms that aren't designed to collect personal data at all. This thread shows that the problem goes back to at least July last year. SORT IT OUT MICROSOFT!

Not sure if I'm doing this right - my first use of this method of asking for help.
I'm writing a test with this question:
It has triggered this response. How do I get around this please? I understand why this type of question can cause problems but the trip-wire seems to be set very low on this function.

Can you work with you admin to get this unblocked?- Will_Carr_SynxCopper Contributor
Yao Ke My admin has had a blockand can't see where to unblock the sharing of my quiz.
Can you send me (or her indeed) a few hints where to look please?

same problem - I am trying to include a short quiz before the next online class (corona...) of a basic computer security course:
any idea how to do it using forms?

Is there a possibility to customize the link for "Terms of Use" in the top bar announcement? (Currently this just brings me to the subscription section of my Office User profile).
- Tech_2000Copper ContributorIt just happened to me as well on one of the forms I was editing. I tried editing the form, refreshed page, logging out and back in to no avail. Please advise a fix for this
