shivamrathore5600 I showed you the link to copy on the screenshot, I can't explain it any more clearly than that. It might be that it's a different problem but making sure you have sent out the collect responses link rather than the edit link is the always the first place to look.

Rob_Elliott - Thanks Rob. That did the trick. I was trying to copy the URL from the wrong location. Thank you for the response.
So was I, the UI could be a lot clearer about where you get the form link from. Thanks
