pwintert there is no way to pre-populate anything in Forms with data from a SharePoint list, spreadsheet etc. There are plenty of requests for it on the Microsoft Feedback Portal https://feedbackportal.microsoft.com/feedback/search/0ab9de26-ff1b-ec11-b6e7-0022481f8472?q=SharePoint+list
