KeithRitner you will need to have 2 flows in Power Automate, one for each shift, with a recurrence schedule trigger and a send an email action with a link to the form.

But how do I get microsoft forms to track those responses by respondent, and maintain anonymity?
For example, i want to measure workflow, stress and knowledge at baseline, at 3 months, at 6 months and at 9 months. I want to track this data in such away that I can track changes at the individual level for statistical analysis. So forms needs to be able to track associate that data at the user level but would keep the data itself anonymous.

You can't do that. A form is either anonymous or it's not. If the form is set to "only people in {organisation} can respond" then make sure you don't give the Collaborate or Duplicate link to anyone as that way only you will be able to see the responses.

Ok, I can get around that with careful planning on the IRB, just designate one individual to handle the data and make it anonymous after the data pull. So the ultimate question is, how can I measure baseline and three follow up measurements in MS Forms? this is something I used to be able to do really easily in platforms like RedCap and Qualmetrics, but at my new place of employment I am stuck with MS Forms.
