Polls & Quiz not loading

Hey,

I'm not sure if anyone else is experiencing this issue but as an organization we're not able to load quizzes or polls from within a meeting. Usually we'd add Forms to the meeting (top bar on the right + app)  and then add questions that way but its not allowing us to do that. We just get a black box with a spinning circle (sometimes)  when trying to add the "forms" app as pictured above.

We're able to add a "quiz" if we try and do it via the "message extension" button but this is only one form of question and doesn't give us full functionality.
