Options for answers disappearing from a live form

I have a very simple Form (survey) open to students with

  • 4 Likert scale type ( 5 choices) questions and then
  • a question to be answered by choosing any number of 13 check boxes.

First problem occurred when one of the questions magically reduced from 5 to 2 choices.

Second time this same thing happened to 2 of the questions plus the check box options halved.

 

Needless to say the data is horribly corrupted and the students are understandably annoyed.

Has anyone else experienced this?

    Hi Donnaconda 

     

    I personally haven't experienced this issue but I will ask an obvious question to confirm, does anyone else have edit access to the form?
