Multiple choice numerical answer question

Let me try to explain what I am trying to do: I would like to have a single question where I have a list of items showcased and the answer I would require is a numerical value that is assigned to the listed item. In the sense that I would like to ask a percentage of a feedstock material that a facility receives and would give 3-4 common options and would like for the people to answer a numerical value corresponding to the showcased predefined answer.

 

I have searched and not found that this is possible with Microsoft forms, even that I think should be relatively simple from the code perspective. Multiple choice answers only accept select the box option and a numerical value is only available in text answers. The not attractive solution I have found is have 5 questions one behind the other that each accepts a numerical value answer, but this is not the most stylistic and clean solution.

 

Is there a way to put it in the form I would like to have this question? Thanks in advance for the time!

    It sounds like you need to select likert question in your form for this. The numerical value selected is saved in the spreadsheet behind the form and can feed into a flow in Power Automate so you can send the responder a customised email with their responses (we always do this at my company).
