Microsoft Forms link showing pre-filled data

Hi all,

In need of some assistance. I have created a form for our company and shared the URL however, when you click the URL, some of the data is pre-filled. I thought I had copied something incorrectly, so did it again and the same thing is happening.

I have never experienced this before and am unsure how to fix what I have done.
