Informations to confirm

Hi all,

I would like to create a form with existing informations in our ITSM to check if users really have the computer we registered.

The form will display computer name list (1 at least or more if possible) and for each computer the user must check Y/N bullet point to confirm what computer he has or not.
Is it possible with MS Forms ? Any example screenshot to share ?

If not possible what will be your choice to automate data processing (no mailing !)

    Thank you for your answer.

    I did not consider this solution because I want to avoid manual data entry errors.
    It seems that it is the only solution and I will implement it.

    A form can't look up any data from a another datasource, so you can't display to the user the types of laptop your other system thinks they have.

    You could have a choice question with a list of makes. Then use branching so if HP is selected it takes the user to another section and choice question for which model of HP laptop they have. If they selected Dell it takes them to another section and choice question for which model of Dell laptop they have. And so on.

    However, the branching is not possible if the first question allows multiple answers. You would have to get the user to enter the models of laptop they have manually.
