Hi Admin, when you submit this form, the owner will be able to see your name and email address.

Hello. Within Sharepoint (filling in a FORM) we got always on top this message for every user

"Hi Admin, when you submit this form, the owner will be able to see your name and email address."

So for every user ADMIN and not the name of the user logged in filling in the form. 

So where can I configure that ADMIN should be the logged in user ?
