Forms quiz marks wrong correct answers.

As you can see in the first image, I marked four of five answers as correct (multiple answers).  However, when marking the exam I found that there are several answers duplicated and some that are correctly answered by my students are automatically marked as wrong (see the second image). Has anyone had a similar problem? Is there anything I can do to sort it out?
