FIll in the blank and check box question combined

I am wanting to know if there is any way possible to layout a question like the following one attached? For this question you would need to fill in the blank and have check boxes available if you do fill in the blank.

    Hannah151 that's not possible with Microsoft Forms but you could build this with Power Apps.
