Excel download of responses not accurate file

I copied an old Form, renamed it, and changed up the questions. Although the respondent does see the correct questions, when I download the responses to excel, columns are included that were from the old form and sometimes duplicated.  And the downloaded excel file is named after the old form as well. Why is this happening?
